Assessment reports
The assessment report matters as much as the chair
Within 24 hours of the visit you get two documents. The assessment report covers your client, what they need and the outcomes they want to achieve, with the full specification behind it. The costs are all in a separate itemised quotation, sent at the same time.

Situation, challenges, desired outcome
Your client’s situation discussed, the functional challenges they face day to day, and the outcome they hope to achieve or regain.
Trial and recommendation
Each item is trialled with the client, and recommendations made to ensure the best solution.
Your quotation
Itemised with equipment specification, delivery, installation and fitting.
What’s inside
Every assessment report, every time
| Section | What it covers | Why it saves you time |
|---|---|---|
| Client and referrer details | The client, and you, including your professional registration number | Goes into your file as it is |
| Situation, challenges and desired outcome | Their situation, the functional challenges they face day to day, and the outcome they hope to achieve | No second visit to gather detail |
| Equipment requested | What was asked for, and the reason for the request | Your brief, in writing |
| Trial and specification | Each item trialled, its specification, and how the trial went | Evidence for your final recommendation |
| Recommendation | The equipment we would recommend, and the reasons behind it | Our view in writing. The final recommendation stays yours |
| Other observations and additional equipment | Anything else we noticed, and what else could help | Options you might not have considered |
No prices in any of that. The costs sit in a separate itemised quotation, sent with the report, so you can pass the quotation straight to a funder.
